Step 1: Set Up Patient Portal
- Click on the green Signup Link in the top menu of the SmartND website. (www.smartnd.ca). This will open up the Registration page.
- Select I Am A Patient at the top, and fill in all the information. All fields are required. Keep in mind that you will not see any information in your patient portal unless the information you enter matches the information that you have provided or will provide your practitioner.
- When you are done, click on the blue Submit button at the bottom. This will direct you back to the login page.
- Enter the e-mail address and password you chose in the above steps to log in.
Once you log into your patient portal, you will need to enter “Access Codes” provided by your practitioner in order to gain access to intake forms or your medical records. If you have already had your visit with your practitioner, you will have been provided a sheet of paper which includes your individual Access Code as well as instructions on how to use it.
If you have not yet had your visit, then Access Codes may have been sent to you via e-mail or text message. Find these codes and redeem them in the Patient Portal in order to gain access to forms and records.
